Do health insurance agents get renewals? How U65 commissions work

Yes, health insurance agents earn renewals. Here is how under-65 health commissions work, what a renewal looks like on a real case, and why vesting matters.

Yes. Under-65 health insurance pays agents a commission in the first year and a renewal commission every year the client keeps the plan. That is the single biggest difference between health and most of what life agents sell today.

Paid once vs. paid every year

On most life cases, you write the policy, collect your commission, and move on. Next January your production starts back at zero.

Health works differently. The client keeps the plan, and the carrier keeps paying you on it. Each year you write new business, the renewals from earlier years are still coming in underneath it. That is what agents mean when they say a health book stacks.

What a case can look like

Commission schedules vary by carrier and product, so treat this as an illustration, not a promise. Take a family paying $7,200 a year in total premium, with a core medical plan plus bundled coverage like dental, vision and hospital indemnity:

    Example
    Core medical plan (about 2/3 of premium)$4,800 × 25% = $1,200
    Bundled products (about 1/3 of premium)$2,400 × 50% = $1,200
    First-year commission$2,400
Renewal each year they keep it (about 5% of premium)$360

One case renewing at $360 is not life-changing. One hundred of them is $36,000 a year arriving whether or not you write a single new policy that year. That is the math behind building a health book.

    Why bundling matters

Notice that the bundled products paid as much in year one as the medical plan, on half the premium. Learning to build a complete package is good for the client, who gets fuller protection, and it roughly doubles what a medical-only sale pays.

This is the skill most new health agents are never taught. Good training should cover it from day one.

The question nobody asks: who owns your renewals?

Renewals are only worth building if they belong to you. Some organizations do not fully vest agents, which means if you leave, your renewals can stay behind. Agents have spent years building a book only to watch it walk out the door.

Before you contract anywhere, ask directly:

    • Am I 100% vested in my renewals from day one?
    • If I leave, do my renewals come with me?
    • Is that in writing in my contract?

If the answer to any of these is unclear, keep looking.

What this means if you sell life today

You do not have to choose. Many agents keep writing life and add health on top, often with the same clients. The life business keeps paying you up front. The health business builds a layer of income that renews underneath everything else.
