How to add health insurance as a life insurance agent

A step-by-step guide for life, final expense and mortgage protection agents adding under-65 health insurance: licensing, contracting, leads, and how the commissions renew.

If you sell life, final expense or mortgage protection, you are already one license line away from a product your clients are actively shopping for. Under-65 health insurance is the easiest add-on in the business to start, and it is the only one of the three that keeps paying you after the first year.

This guide walks through exactly what it takes, in order.

Why life agents are adding health right now

In life insurance, you create the need. Nobody wakes up wanting a term policy, so every sale starts with convincing someone to care.

Health is the opposite. Families shop for it because they have to. Premiums keep climbing, and many people who lost subsidy help are looking for private options. The prospect is already motivated when you call.

The second reason is the money. A life case pays you once. A health case pays you in year one and again every year the client keeps the plan. Write steadily for a few years and your renewals start carrying real weight in your income.

Step 1: Get the accident and health license line

You need a health license to sell health. Many states issue a life-only license, so check yours before anything else. Your state's Department of Insurance lookup will show your lines, or search your name on the NIPR site.

If you only have life, the health line is usually a short pre-licensing course plus a state exam. Most agents finish it in a couple of weeks. Course and exam fees vary by state, so check your state's requirements.

Already licensed in several states for life? Add the health line in those states too. Health leads are national, and more states means more people you can help.

Step 2: Choose who you contract through

You do not contract with health carriers on your own. You go through an FMO (field marketing organization) that holds the carrier relationships and gets you appointed.

The FMO you pick decides most of your experience: which carriers you can sell, whether you own your renewals, what training you get, and whether you have leads and tools on day one. Ask every FMO these questions before you sign:

    • Am I 100% vested in my renewals from day one?
    • Which carriers will I be appointed with, and how long does it take?
    • What does training look like for someone who has never sold health?
    • Are there leads available, and are they exclusive or shared?
    • Do I pay for the CRM, dialer or any platform fee?

Step 3: Learn the products you will actually sell

Under-65 health is not one product. Most families end up with a core medical plan plus supporting coverage: dental, vision, hospital indemnity or accident.

Learning to build that package is the biggest skill shift for a life agent. The client gets fuller protection, and you get paid on every piece of it. Good training should teach you which plan fits which situation, not just how to run a quote.

Step 4: Start working leads by phone

Health is sold virtually, by phone. If you can run a life appointment over the phone, the same skills carry straight over: rapport, fact-finding, handling objections and closing.

The difference is the conversation starts easier. You are not convincing someone to think about dying. You are helping someone who already called about a problem they have right now.

Step 5: Keep selling life, often to the same client

Adding health does not mean giving up life. The homeowner you just wrote mortgage protection for may need health coverage too. The family shopping for health may need life insurance they never got around to.

Agents who offer both have more reasons to call, more to offer on every call, and a book that pays them every year.
